The following documents are acceptable forms of proof of merger/acquisition:

1. State or FCC filings identifying the company(ies) being merged/acquired and the company it is being merged into or acquired by and the effective date of the merger/acquisition
2. Merger/Purchase Agreement that clearly identifies the company(ies) being merged/acquired and the company it is being merged into or acquired by and the effective date of the merger/acquisition
3. Certificate of Merger/Articles of Merger filed with the state
4. Screen shot of the FCC 499 page for the company showing the “Replaced by filer” number and the referenced acquiring companies FCC 499 page
