1. Business licenses that identify the company as a sole proprietorship
2. Screen shots from state or city websites that identify the company as a sole proprietorship
3. Certificate of assumed name (Doing Business As) with the business name and the legal contact’s first and last name
4. Letter from an attorney or accountant on letterhead that confirms the company is a sole proprietorship
