1. Per NANPA records, the numbering resource is not assigned to the OCN reported on the form as of December 31 for the February 1 NRUF reporting period and June 30 for the August 1 NRUF reporting period. Service Providers should refer to the Part 3 approval date rather than the effective date to determine if the resource must be reported if it was recently returned, OCN changed or resource was transferred.

2. If you are the CO code holder reporting on a pooled code, you must report on all ten thousands-blocks. For any thousands-block that has been donated to the pool and is no longer assigned to or retained by your OCN, enter an “X” in the “Donated to Pool” column on the U1 form.  It is also recommended that you report utilization for any contaminated thousands-block that has been donated to the pool.

3. If you are not the CO code holder and the NPA-NXX or NPA-NXX-X is not assigned to your OCN, but TNs were assigned to your OCN by the CO code holder or thousands-block holder as “intermediate” numbers, you must report the NPA-NXX or NPA-NXX-X on the applicable U3 or U4 form.
